Thank you for choosing The Lay Effect Aesthetics. To maintain a high standard of care, professionalism, scheduling integrity, and respect for all clients, the following policies are in place. CONSULTATION POLICY All new clients are required to complete a consultation prior to treatment. Consultations are intended to assess facial anatomy, aesthetic goals, medical history, treatment candidacy, and overall clinical appropriateness. • Consultation Fee: $75 • Consultation fees are non-refundable • Consultation fees are applied toward treatment completed within 30 days of the consultation date Treatment recommendations and candidacy are determined solely based on clinical assessment and medical appropriateness. TREATMENT TIMING Recommended treatment plans should be initiated within 60 days of the consultation unless otherwise discussed with your provider. Treatment delayed beyond this timeframe may require reassessment prior to proceeding. PAYMENT POLICY Payment is due in full at the time services are rendered. Pricing is based on treatment performed, product utilized, provider assessment, and individual treatment needs at the time of service. Accepted payment methods are available at checkout. CARD ON FILE & DEPOSIT POLICY A valid card on file is required to secure all appointments. A non-refundable deposit is required at the time of booking and is applied toward the scheduled service total when the appointment is completed as scheduled. Appointments canceled or rescheduled with at least 24 hours’ notice may transfer the deposit one time toward a future appointment scheduled within 60 days of the original appointment date. Appointments canceled, rescheduled, or missed with less than 24 hours’ notice will result in forfeiture of the deposit and a $50 late cancellation fee charged to the card on file. Failure to appear for a scheduled appointment without notice will be treated as a no-show and subject to the same forfeiture and cancellation fee policies. By booking an appointment, you authorize The Lay Effect to charge the card on file for: • Required deposits • Late cancellation fees • No-show fees • Applicable fees in accordance with these policies Two-week follow-up appointments do not require a deposit; however, appointments canceled, rescheduled, or missed with less than 24 hours’ notice will incur a $50 late cancellation fee charged to the card on file. LATE ARRIVAL POLICY Please arrive on time for your appointment. Late arrivals may result in a shortened appointment or the need to reschedule based on provider discretion and scheduling availability. If rescheduling is required due to late arrival, forfeiture of the deposit and applicable cancellation fees may apply. REFUND POLICY All services and treatment fees are final. Due to the nature of medical aesthetic treatments and individual biological variability, specific outcomes cannot be guaranteed. Refunds are not provided for services rendered, products used, or treatment dissatisfaction. ADDITIONAL TREATMENTS Some clients may require additional treatment sessions, maintenance treatments, or touch-ups to achieve or maintain desired aesthetic outcomes. Unless explicitly stated otherwise in writing, additional treatments are not included in the original treatment cost and will incur additional fees. RIGHT TO REFUSE OR DISCONTINUE TREATMENT The Lay Effect reserves the right to refuse, postpone, or discontinue treatment at any time if treatment is determined to be unsafe, medically inappropriate, outside provider scope, or not in the client’s best interest. POLICY ACKNOWLEDGMENT By scheduling, booking, or receiving treatment through The Lay Effect, you acknowledge that you have read, understood, and agreed to the policies outlined above.
